Katalyst PT provides physical therapy, strength training, wellness, and performance-based services including but not limited to: rehabilitation therapy, adult fitness training, youth athletic performance training, golf performance, and wellness-related educational content. Services may be delivered in person, online, or via third-party platforms under our discretion.

THE WEBSITE AND ANY INFORMATION PROVIDED THEREIN ARE FOR EDUCATIONAL AND INFORMATIONAL PURPOSES ONLY AND DO NOT CONSTITUTE MEDICAL ADVICE.

Katalyst PT does not intend for any material on this site to be used for medical diagnosis or treatment. 3. No Provider-Patient Relationship

No provider-patient relationship is created by your use of the Website or submission of any forms or communications. A formal provider-patient relationship is established only upon mutual agreement after an in-person evaluation or telehealth intake by a licensed clinician employed or contracted by Katalyst PT.

4. Scheduling, Payments, Cancellations, and No-Show Policy

All appointments must be scheduled through our approved booking channels. You agree to provide accurate, current, and complete information when scheduling services.

Payments: You agree to pay all fees and charges associated with the services as described at the time of booking or as otherwise communicated. Payments may be processed through third-party platforms and are subject to those providers’ terms and security protocols.

Cancellations: You must provide at least [24 hours] advance notice to cancel or reschedule appointments. Late cancellations and no-shows may be charged up to the full service fee at the discretion of Katalyst PT.

Refunds: Service fees are non-refundable unless otherwise stated. Membership fees, prepaid packages, or class passes may be governed by separate cancellation and refund policies specific to those offerings.

5. Memberships and Packages

Memberships, class packages, and bundled services may be subject to additional terms and conditions, including minimum term requirements, auto-renewal provisions, early termination fees, and usage limitations. By purchasing a membership or package, you agree to any supplemental terms provided at the time of purchase.

6. Insurance and Billing

Katalyst PT may operate on an out-of-network or cash-pay basis. You are responsible for understanding your insurance benefits and for submitting claims independently if you choose to do so. We make no guarantees regarding reimbursement from your insurer. Any assistance we provide in submitting claims is offered as a courtesy only.

7. User Conduct

You agree not to:

Use the Website or services for any unlawful purpose or in violation of applicable local, state, or federal law;
Transmit any viruses, malware, or other harmful code;
Attempt to gain unauthorized access to any systems or networks related to the Website;
